This template includes eight pre-configured fields:
Customer Name (Short Text)
Full name for order processing and shipping labels. Used for invoices, delivery confirmations, and customer communication.
Email Address (Short Text)
Contact method for order confirmations, quotes, shipping updates, and customer service. Essential for the order fulfillment workflow.
Product or Service (Short Text)
Free-text field where customers describe what they want to order or request a quote for. Flexible for businesses with varied offerings or custom solutions.
Quantity (Short Text)
Number of units or volume needed. Helps with pricing, inventory checks, and production planning. Examples: "10 units", "5 licenses", "3 months".
Additional Options (Checkboxes)
Four common upsell options: Express processing, Custom branding, Priority support, and Extended warranty. Allows customers to self-select premium features.
Delivery Preference (Dropdown)
Four shipping options: Standard, Express, International, and Local pickup. Helps logistics teams route orders correctly and set customer expectations.
Additional Notes (Long Text)
Free-form field for special instructions, customization requests, or questions. Captures order details that don't fit structured fields.
Agreement (Checkboxes)
Terms and conditions consent checkbox. Ensures customers acknowledge your sales terms, return policy, or custom order agreements before submission.

How to use this template
- Navigate to your form in the dashboard
- Click the Fields tab
- Click Use Template in the empty state
- Select Order / Quote Request from the template grid
- Customize the Additional Options checkboxes to match your offerings
- Click Save to apply the template to your form

Customizing the template
Adapt the template to your order workflow:
Add pricing tiers
Include a Dropdown: "Pricing Tier" or "Package" with your product options: Basic, Professional, Enterprise. This is cleaner than free-text for standardized offerings.
Include delivery address
Add Short Text fields for "Shipping Address", "City", "Postal Code", and "Country" if you need address details upfront. Alternatively, collect these via email after the initial quote.
Add payment method preference
Include a Dropdown: "Preferred Payment Method" with options like Credit Card, Bank Transfer, Net 30 Terms, PayPal. Helps with invoicing and payment processing.
Expand product selection
Replace the free-text "Product or Service" field with a Dropdown or Checkboxes listing your specific products if you have a defined catalog.
Add delivery date
Include "Requested Delivery Date" (Short Text) for time-sensitive orders or events. Helps with production scheduling and setting expectations.
Customize additional options
Replace the default checkboxes with options specific to your business: "Gift Wrapping", "Rush Production", "Setup/Installation", "Training Included", etc.
Best practices
Respond with quotes quickly: Customers comparing vendors will go with whoever responds first. Aim to send quotes within 24 hours, or set expectations in your form description.
Use notes field for custom orders: Encourage customers to be specific in Additional Notes. Good prompt: "Please describe any custom specifications, colors, sizes, or special requirements."
Clarify quote vs order: In your form title and description, specify whether this is a binding order or a quote request. Managing expectations prevents confusion.
Calculate shipping before confirming: If delivery costs vary significantly (international, heavy items), make it clear the initial quote may not include final shipping costs. Follow up with accurate pricing.
Offer multiple contact methods: Some customers prefer phone for large or complex orders. Include your phone number in the form description or confirmation email.
Track conversion rate: Monitor how many quote requests convert to actual orders. If conversion is low, your quotes might be too slow, too expensive, or lacking crucial information customers need.
